In return for being allowed to participate in TG31 MIM Foundation, Inc. volunteer activities and all related activities, including any activities incidental to such participation ("Volunteer Activities"), the undersigned Volunteer or Parent/Legal Guardian of Volunteer if Volunteer is under age 18 (hereafter referred to using "I", "me", or "my") releases and agrees not to sue TG31 MIM Foundation, Inc. or its officers, directors, employees, sub-contractors, sponsors, agents and affiliates ("the Foundation") from all present and future claims that may be made by me, my family, estate, heirs, or assigns for property damage, personal injury, or wrongful death arising as a result of my participation in the Volunteer Activities wherever, whenever, or however the same may occur.

I understand and agree that TG31 MIM Foundation, Inc. is not responsible for any injury or property damage arising out of the Volunteer Activities, even if caused by their ordinary negligence or otherwise.

I understand that participation in Volunteer Activities involves certain risks, including, but not limited to, serious injury and death. I am voluntarily participating in the Volunteer Activities with knowledge of the danger involved and I agree to accept all risks of participation.

I also agree to indemnify and hold harmless TG31 MIM Foundation, Inc. for all claims arising out of my participation in the Volunteer Activities.

I understand that this document is intended to be as broad and inclusive as permitted by the laws of the state in which the Volunteer Activities take place and agree that if any portion of this Agreement is invalid, the remainder will continue in full legal force and effect.

I also acknowledge that TG31 MIM Foundation, Inc. has not arranged and does not carry any insurance of any kind for my benefit, or that of my parents, guardians, trustees, heirs, executors, administrators, successors, and assigns.

I represent that, to my knowledge, I am in good health and suffer no physical impairment that would or should prevent my participation in Volunteer Activities.

All matters relating to this Agreement shall be governed by the laws of the State of Georgia. Any claim arising under this Agreement shall be brought exclusively in the state or federal courts located in Gwinnett County, Georgia, to which I hereby consent to personal jurisdiction.

BY SIGNING, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD ALL OF THE TERMS OF THIS AGREEMENT AND THAT I AM VOLUNTARILY GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.

I desire to volunteer for TG31 MIM Foundation, Inc. and engage in the activities related to being a volunteer, which may include but are not limited to: youth football camp coaching assistance, event setup and breakdown, registration coordination, hydration and snack distribution, photography, and general camp operations.

I hereby freely and voluntarily, without duress, execute this Release under the following terms:

1. Waiver and Release: I, the Volunteer, release and forever discharge and hold harmless TG31 MIM Foundation, Inc. and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my volunteer work.
2. Insurance: I understand that TG31 MIM Foundation, Inc. does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance of any nature in the event of my injury, illness, death, or damage to my property.
3. Medical Treatment: I hereby release and forever discharge TG31 MIM Foundation, Inc. from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my time as a volunteer.
4. Photographic Release: I grant and convey to TG31 MIM Foundation, Inc. all right, title, and interests in any and all photographs, images, video, or audio recordings of me or my likeness or voice made by TG31 MIM Foundation, Inc. in connection with my volunteer provided services.
5. Confidentiality: I understand that in the course of my volunteer work I may have access to confidential information regarding camp participants, their families, and foundation operations. I agree to keep all such information strictly confidential and to not disclose, share, or discuss this information with any unauthorized persons during or after my volunteer service.
6. Code of Conduct: I agree to conduct myself in a professional and appropriate manner at all times while volunteering, to follow all instructions given by camp staff, and to prioritize the safety and well-being of all camp participants. I understand that TG31 MIM Foundation, Inc. reserves the right to dismiss any volunteer whose behavior is deemed inappropriate or inconsistent with the values of the organization.
7. Other: I expressly agree that this Release is intended to be as broad and inclusive as permitted by law, governed by the laws of the State of Georgia. I agree that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release.

BY SIGNING, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD ALL OF THE TERMS OF THIS RELEASE AND THAT I AM VOLUNTARILY GIVING UP SUBSTANTIAL LEGAL RIGHTS.

Because TG31 MIM Foundation, Inc. serves minor children (ages 6–16), the safety and protection of every participant is our highest priority. All adult volunteers (age 18 and older) are required to consent to a background screening as a condition of volunteer participation.

Background Check Authorization: I hereby authorize TG31 MIM Foundation, Inc. to conduct a background check, which may include criminal history records, sex offender registry searches, and any other screenings deemed appropriate by the organization. I understand that the results of this background check will be used solely to determine my eligibility to volunteer and will be kept confidential.

Child Protection Commitment: I agree to abide by the following child protection standards while serving as a volunteer:

1. I will not be alone with a minor in an unsupervised or secluded setting at any time during camp activities.
2. I will immediately report any suspected abuse, neglect, or inappropriate behavior toward a minor to camp leadership.
3. I will not exchange personal contact information (phone number, social media, etc.) with any minor participant without express permission from camp leadership and the minor's parent/guardian.
4. I will not photograph or record minor participants on personal devices without authorization from camp leadership.
5. I will treat all participants, families, staff, and fellow volunteers with dignity, respect, and kindness at all times.

Disclosure: I certify that I have never been convicted of, nor pled guilty or no contest to, any crime involving violence, abuse, neglect, or exploitation of a minor. I understand that any false statement or omission on this form may result in immediate dismissal from volunteer service.

All matters relating to this consent shall be governed by the laws of the State of Georgia. This consent remains in effect for one (1) year from the date signed below and applies to all TG31 MIM Foundation, Inc. events during that period.

BY SIGNING, I AUTHORIZE THE BACKGROUND CHECK AND AFFIRM THAT ALL INFORMATION PROVIDED IS TRUE AND ACCURATE TO THE BEST OF MY KNOWLEDGE.
